Val,
With all the problems I've had with Jax and the seven dog trainers I've had in his eight years of life, I've learned a lot. 
Most dog's bad habits are our fault.  We give in before they will. As difficult as it may be, ignore her.  She knows how to get on bed.  Let her get up by herself, or put treats on the ramp.  In the end, when she gets there by herself-praise her until you seem stupid. 
Award the positive, ignore the negative.  My two cents.
