Can't give you too much help but IMHO it narrows down to choice of #1 or #4. 

P.S. Please don't flame me...I believe it's not right or wrong to choose to release a companion animal from suffering at any point along the way.  We were given the responsibility to teach us about love and loss. You just have to live with your choice afterward...J.
