I bought collars for my dogs at www.orvis.com.  They are well made and they 
embroider the dogs name and your phone number on them.  I don't know if you 
can get other things put on the collars or not.

I did order the wrong size to begin with and they took them back no 
questions asked and got me the right size. When ordering them, it is better 
to air on the side of smaller than bigger as you want the information to be 
easy to read.  If you buy it too big and adjust it, the info can become 
lost.
