There are plenty of embroidery shops on the Internet that sell dog- 
breed polo shirts. Just Google something like "embroidery" "dog  
breed" "polo shirt" and you'll find several.

If you're giving the shirt as a gift, I would make sure that the  
design is embroidered rather than transfer-printed (like a T-shirt),  
because a printed design won't stand up to laundering as well.

Don't know if any rescues are currently selling embroidered shirts,  
but Michigan Basset Rescue's website is currently selling baseball  
caps with embroidered basset designs.
