Have you have her eyes checked? Is it possible she is worried and afraid
because especially at night, even with the TV or lights on, she cannot see?
I would start with your Vet and have Tilly's eyes looked at, and/or try a
canine opthamologist.
It's just a thought.
If it is canine dementia, then there are drugs specific to that which might
help.  Does she sleep in the bed or room with you? Would she feel safer in
a crate? (I am just throwing out thoughts...) Probably you have tried all
this (it sounds as if you have tried a lot.) Try everything you can within
reason. I am a big believer in going to the Universities, but not everyone
is.
