Small steps (increments) and positive re-enforcement.
 
Long walks on leash ... the moment she pee's "good girl" luvin's treats etc
.. try not to do too much talking or even give much attention to her before
she pee's or she may see the reward as just being with you and that's what
we're trying not to have her do.. ie.. outside she's happy go lucky just to
be by your side.. the moment she doesn't have your attention inside, she
pees as a "hey I'm here luvsies me"
 
Make it all about the pee.. until she pees.  Then you could talk and chatter
with her about the weather or the dust never settling but not until *after*
she's relieved herself.  If you have a dog park in your area ... you could
walk the perimeter on leash until she's peed and then she gets off leash
time, etc.. 
 
Similarly after doing this on walks for a bit.. then start walking her
around your yard ... don't let her off leash until after she's peed.  And
again with the treats/rewards etc.. 
 
Once you are comfortable with her peeing relatively quickly to get the
treat.. you can take her outside off leash to try and see if she'll pee...
etc.. but always with positive reinforcement ... 
 
once you have her going outside off leash in your presence then its time to
move onto trying to get her to walk on her own some, while you stand in one
area waiting for her to go.. then praise, treats, etc.. 
 
Then move on to use the doggie door (or out with the pack) without you ...
and as soon as she goes you could call her in/over and praise/treat/etc... 
 
small gradual steps.. building upon what she's done right prior.. so that
you're constantly reinforcing the positive behavior.. 
 
and she's not "old" she's just Temporally Well Acclimated ... Stormy
Weathered ... She is filled with the Stone Washed Comfortablilty... :-)

Okay,I've had Dolly for a week now.In case you don't remember,she was left
at my vet to be euthanized because she peed in the house. 
This is an 8yr. old spayed female and all of the tests that the vet has run
indicate this is behavioral.
Tonight I had her outside for twenty minutes watching her and waiting for
the pee.She just stands there and calmly looks at me.I finally gave up and
came in and she peed next to me while I was busy. 
When we're out she doesn't seem to comply,and I'm not sure how to get my
point across to an older woman. 
How does one get an older woman to get with the program and not upset her.
