First and foremost start on a hypoallergenic food.  I would not even mess with over the counter stuff at this point - go right to Hill's z/d or something similar.  Then seek out a dermatologist.  Skin problems are long draw out expensive problems but once the underlying problem is addressed you should be good.

At this website you can search dermatologists in your area.  It might be expensive initally but going back to the regular vet over and over again with no resolution will probably be more expensive. 

Also someone just posted something the other day about TheraDerm or something like that that was a food supplement.  Anything with Omega 3's is good for decreasing inflammation.
