You did not say what you are feeding your dog.  Check to be sure that  
there is no soy in any form in what you are feeding.  Also, corn, wheat and  
gluten can cause allergic reactions that show up as "hot spots".
Quite frequently, food allergies are the cause of hot spots, and the above  
things are some of the more common ones.
