Sandy,
    This sounds like a bacterial skin infection.  The dog can have no symptoms other then a horrible, recurring odor.  When I first adopted Beau, I thought he just stunk from having been kept outside for four years.  He had no hair loss, no scabs, no hotspots, nothing just a horrible odor.  My vet cured this with antibiotics and a antibacterial shampoo.  It may take a while and some twice a week baths but it eventually cleared up entirely.
