Hi Marlene,
    Extreme lethargy are side effects of hydroxyzine.  If it is working, I hesitate to ask the vet to change.  Zyrtec and Claritan minus the decongestant can both be used on dogs for itching and are available over the counter.  I don't know if your girl could try either of these instead.  My vet has had me use both.
