Our 13 year old Buddy has had an ongoing, unrelenting ear infection. 
We tried 4 different drops and different ear cleaners and saw the 
alternate vets at our facility in case they had any new ideas. 
Buddy's on antibiotics for life for a different problem, and one vet 
suggested that could be contributing to the yeast. So at his 
suggestion, we added yogurt to the milk and metacam he gets in the 
morning; nothing helped. Finally, we had a culture done; the main 
culprit was a resistant staph infection. The culture didn't respond 
to the first 20 drugs tested against it. The 21st drug was taken off 
the market because it caused aplastic anemia in humans; the 22nd drug 
isn't made but our vet was able to have it compounded for us from a 
pharmaceutical company in New York. (We're in California.) Buddy got 
different ear drops, too. I could hardly stand to ask the cost of the 
new drug. It was only $40 (and $41 for the drops). I suspect the vet 
had a bunch made to keep in stock, and that kept our price down.

Buddy's healing! After less than a week on the new regime, no more 
black gunk in his ears. Hallelujah!
